These tasks could be support with showering (perhaps needed after a recent fall), shopping and meal preparation due to a reduction in mobility and dexterity. Day-time care also can include a higher level of support such as medication management and hoisting.

Clients that use our night-time care service are unable to be left alone during nights. Examples of common tasks carried out at night include medication administration or continence management. This service requires care and support staff to stay awake and fully alert.

With our sleep-in care service your care and support worker will snooze in a chair or on a sofa until they are needed. This option is recommended for clients that don’t require constant monitoring during the night as it substantially lowers the cost.

Essentially, this a combination of 12 hours per day of day-time care and 7 nights per week sleep-in care but with a significant reduction in cost. During your care and support workers scheduled time-off, HPC will provide a replacement staff member to ensure there is no gap in our service.

HPC will place a small team of care and support workers to provide round-the-clock care. This is the most intensive level of support that HPC offers and is often for clients needing a high level of assistance.

Respite care could be for an afternoon, a week or however long the break is needed for. Respite is often used by families looking to go abroad and who are unable to care for their loved one for a short time.

The Care Tiers

For every service HPC offers, there are 3 levels of care the HPC can provide. Each tier involves a higher level of training for care and support workers, an increased involvement from management and more frequent supervision. Care tiers ensure that the right level of care and support is provided to ensure that our service is person centred, effective and safe. Care tiers also allow services to be priced fairly so that clients pay the correct amount for their service.

Tier 1

Including personal care, continence care, housekeeping, meal preparation.

TIER 2

Including Tier 1 care and specialisations such as dementia care, reablement and end-of-life care.

TIER 3

Includes tiers 1 and 2 as well as delegated nursing tasks.

What’s the process?

To determine what service is required you will need to be assessed by a HPC representative. An assessment is a chance for you and your family to let us know what type of service you would like and how you would like your service to be provided. The result of the assessment will be a ‘care plan’, which is your instruction to us of how you would like your service to be provided.

If your care is being provided by a local authority or the NHS then you would have already have been assessed. HPC will receive these details when we are commissioned and this will make up part of your care plan. With commissioned care there are limitations to what type of service we can provide as this is decided by the commissioning body. However, you still have complete control over how you would like your service to be provided. For this reason we will still carry out an assessment but the process will be a lot quicker!
